Negative sentences: no + present + desde hace — In negative sentences with ongoing states, use the present tense: No como carne desde hace dos años (I haven't eaten meat for two years). English uses a perfect tense here; Spanish stays in the present. The negation makes it clear the action has not been happening.

Questions with cuánto tiempo hace que — To ask how long something has been going on: ¿Cuánto tiempo hace que vives aquí? or ¿Desde cuándo vives aquí? Both are natural. To ask how long ago: ¿Hace cuánto llegaste? The answer uses the same hace/desde hace structure.

Llevaba + gerund — the past equivalent — For ongoing actions in the past (before another past event), Spanish uses llevaba + gerund: Llevaba tres años viviendo allí cuando lo conocí (I had been living there for three years when I met him). This is the past-tense equivalent of the hace/desde hace construction.
